    The role of student subjectivity in the professionalism education of medical students

    In the professionalism education of medical students, the education mode of student subjectivity can increase the self-identity, consciously complement the deficiency of the classroom teaching. During these process, students can also reevaluate theirselves and get a more deeply understanding of the professionalism. But this kind of education mode is content selected because of the students’ subjective, and with the problem of continuity and repeatability, it can’t get instead of the classroom education.
    Sodium hydrosulfide inhibits inflammatory factor secretion in macrophages

    Objective: To find out the possible effect of hydrogen sulfide (H2S) on inflammatory factor secretion in macrophage, which might help us to understand the mechanism by which H2S suppresses atherosclerosis. Methods: THP-1 monocytes were induced to differentiate into macrophages by incubation with PMA for 24 h. Then the macrophages were divided into four groups randomly: control group, oxidized low density lipoprotein (ox-LDL)50 mg/L group, ox-LDL+ sodium hydrosulfide(NaHS) 100 or 500μmol/L group. Cells in the control group were treated with neither ox-LDL nor NaHS; cells in the ox-LDL group were treated with 50 mg/L ox-LDL; cells in the ox-LDL+ NaHS 100 μmol/L group were treated with both 50 mg/L ox-LDL and 100 μmol/L NaHS; and cells in the ox-LDL+ NaHS 500 μmol/L group were treated with both 50 mg/L ox-LDL and 500 μmol/L NaHS. Cell supernatants in all the above groups were collected after induction for 48 h. ELISA was used to examine the content of tumor necrosis factor-ɑ (TNF-ɑ), interleukin-10 (IL-10) and macrophage migration inhibitory factor (MIF) in cell supernatant. Results: Compared to the control group, the content of pro-inflammatory factors-TNF-α and MIF rose significantly (P<0.05)while the content of anti-inflammatory factor IL-10 decreased significantly in the ox-LDL group(P<0.05); compared to the ox-LDL group, the content of TNF-α and MIF decreased greatly(P<0.05) while the content of IL-10 increased significantly in the ox-LDL+ NaHS 100 or 500μmol/L group (P<0.05). Conclusion: NaHS inhibits the secretion of pro-inflammatory factors-TNF-αand MIF but promotes the secretion of anti-inflammatory factor- IL-10 with the stimulation of ox-LDL.
    Differentially expressed proteins in the primary focus of nasopharyngeal carcinoma in different clinical types

    Objective To explore the differentially expressed protiens of the tumor tissues of nasopharyngeal carcinoma (NPC) primary focus in different clinical types including upward progressing type,downward progressing type and mixed progressing type. Methods The total proteins of tissues from normal nasopharyngeal mucosa and NPC in different clinical types were separated by two-dimensional electrophoresis (2-DE), then proteins spots were analyzed and the differentially expressed proteins were identified by matrix assisted laser desorption/ionization time-of-flight mass spectrometry(MALDI-TOF-MS) combined with bioinformatics analysis. Results Thirty-one significant differentially expressed proteins between mixed progressing type of NPC and normal tissues were found. And among of them, 18 proteins were up-regulated and 13 proteins were down-regulated in mixed type of NPC. Five proteins of ten differential expressed proteins selected from mixed progressing type of NPC were identified by MALDI-TOF-MS. Heat shock cognate 71 kDa protein,alpha-1-antitrypsin and macrophage-capping protein were up-regulated and Protein S100A9 and Protein 4.1 were down-regulated in mixed progressing type of NPC as compared with those in normal nasopharyngeal mucosa tissues. Thirty-one significant differentially expressed proteins between upward and downward progressing types of NPC were also found. And among of them, 15 proteins were up-regulated and 16 proteins were down-regulated in upward progressing type of NPC as compared with those in downward progressing type of NPC. Six proteins of ten differential expressed proteins selected from upward progressing types of NPC were identified by MALDI-TOF-MS. Mitochondrial aconitate hydratase , alcohol dehydrogenase [NADP+] and Rab GDP dissociation inhibitor beta were up-regulated and NM23-H1, nuclear chloride channel-27 and aldehyde dehydrogenase X were down-regulated in upward progressing type of NPC as compared with those in downward progressing type of NPC. Conclusion There were significant differences at proteins related to tumor metastasis, energy metabolism,retoxity and anti-metastasis between different clinical type of NPC. These proteins may be as markers of clinical types of NPC.
    Alpha-linolenic acid intake attenuates myocardial ischemia/reperfusion injury in diabetic rats

    Objectives to investigate the effects of Alpha-linolenic acid (ALA) intake on myocardial ischemia/reperfusion (MI/R) injury in diabetic rats. Methods The high-fat diet-fed streptozotocin (HFD-STZ) rat model was developed. Age-mathched normal and HFD-STZ rats were randomly assigned to receive non-ALA diet or ALA (o.g., 500 μg/kg per day). After 4 wks of feeding animals were subjected to 30 min of myocardial ischemia and 4 or 6 hrs of reperfusion. Results HFD-STZ rats showed more severe myocardial injury. Although ALA intake for 4 wks did not change myocardial injury in normal rats, it significantly reduced infarct size, plasma creatine kinase and lactate dehydrogenase activities, and apotosis in HFD-STZ diabetic rats. Moreover, myocardial PI3K expression and Akt phosphorylation were increased by ALA intake in diabetic but not normal rats. Conclusion Chronic ALA intake confers cardioprotection in MI/R in diabetic but not normal rats, which is possibly through PI3K-Akt-dependent mechanism.

    Objective To study effect and the mechanism of As2O3combined with recombinant canstatin protein on tumor vascular endothelial cell proliferation, migration, angiogenesis and apoptosis.Methods The cells were induced to differentiate into tumor-derived endothelial cells ( Td-ECs) by co-culturing with supernatants of HepG2 cells. The anti-effect of As2O3 and canstatin on Td-ECs was detected with cell migration, flow cytometry, blood vessel formation assay. Results As2O3 ,canstatin and the combined all effected on Td-EC in vitro inhibited their migration cells respectively: 32.60±1.51、26.60±1.14、21.00±1.87,significantly higher than control group 50.60±2.30(p<0.01),and promoted apoptosis, blood vessel formation.The effect of combined was more significant.Conclusion As2O3 combined with recombinant canstatin protein in vitro inhibits Td-EC proliferation, migration, angiogenesis and promotes apoptosis.

    Objective To investigate the clinical characteristics of lupus patients with positive antiphospholipid antibody (APL) and acute histopathologic lesion of antiphospholipid antibody associated nephropathy (APLN). Methods Data of 100 cases of Systemic Lupus Erythematosus (SLE) admitted to Peking Union Medical College Hospital from 2000 to 2005 was investigated by retrospective analysis. 7 cases of lupus patients with positive antiphospholipid antibody (APL) and acute lesion of APLN were included in the study group. The control groups were divided into control group 1 (7 cases of lupus patients with negative APL and acute lesion of APLN~like renal lesions) and control group 2 (86 cases of lupus patients without APL or APLN). Results The blood pressure and the serum creatinine at biopsy and two years later in patients in the study group were 156±13/92±9mmHg, 159±88μmol/L and 114±60μmol/L, respectively. The prevalence of high blood pressure and high serum creatinine were 85.7% and 42.8%, respectively. The blood pressure including systolic and diastolic blood pressure and mean arterial pressure, the serum creatinine level at biopsy and two years later, the prevalence of renal insufficiency were significantly higher in the study group compared with the control group 2(P<0.05). However there were no significantly different between the study group and the control group 1. Conclusions Lupus patients with APL and acute histopathologic lesion of APLN had higher blood pressure, higher serum creatinine level and poor renal prognosis.

    Objective To detect the membrane location of endogenous PS1 on live cells. Methods HEK293 and Hela cells were transfected with fusion proteins by attaching GFP-PS1.The cellular location of endogenous and exogenous PS1was characterized with antibody by indirect immunofluorescence technique. Results Endogenous and exogenous PS1 was found locating on the nuclear membrane and a little and inhomogeneous distribution in cytoplasm. Conclusions Endogenous PS1 located on the outer membrane of the unfixed and live cells.

    Objective to determine the value of preoperative high-sensitivity C-reactive protein (hsCRP) in predicting perioperative cardiac events (PCE) in elderly patients with coronary artery disease (CAD) undergoing non-cardiac surgery. Methods A prospective cohort study was conducted. 81 patients with the diagnosis of CAD undergoing major non-cardiac surgery were enrolled. hsCRP concentration was measured before surgery. Clinical variables were evaluated. The PCE were followed by the examinations of 12-lead ECG and Troponin I (cTnI) levels. Results PCE occurred in 32% of the patients. Using receiver operating characteristic (ROC) analysis, hsCRP predicted PCE with the area under the curve of 0.75(95%CI, 0.64-0.87; p<0.05). The cutoff value was 12mg/L, showing a sensitivity of 30.77% and specificity of 94.55%. Multivariate analysis revealed that hsCRP>12mg/L as independent predictor for PCE (RR, 26.29; 95%CI, 2.46-281.42; p<0.01), and had higher predictive accuracy than Revised cardiac risk index (RCRI). Conclusion In elderly patients with CAD undergoing non-cardiac surgery, preoperatvie hsCRP is an independent predictor of PCE. hsCRP> 12mg/L identified patients at higher risk.

    Objective To study the effects of Vitamin E Succinate(VES) on proliferation, apoptosis and expressions of associated protein of human esophageal carcinoma cell line Eca-109.Methods Experimental groups were VES of different contrations and control group was culture medium in same volume. After Eca-109 cells were exposed for 24h,48h and 72h, MTT was used to measure the proliferation of cells.Morphological changes of Eca-109 cells were observed by transmission electron microscopy. Apoptotic rate and the expression of Survivin, Caspase-3 protein of Eca-109 among different concentration groups was examined by flow cytometry. Results The inhibitory rate increased gradually,with the increasing concentration of Vitamin E Succinate and prolonging of treatment time(P<0.01).Apoptosis morphous change appeared in different degree in cells. With the increasing concentration of VES,the apoptotic rate of every treatment group was significantly higher than that of control group(P<0.05). The protein expression of Survivin in Eca-109 reduced graduately(p<0.05),but Caspase-3 increased graduately(p<0.05). These two proteins had inverse correlation(r=-0.95, P <0.05). Conclusion: Vitamin E Succinate can inhibite the proliferation and induce apoptosis of Eca-109 cell line.The effect may be ralated to up-regulation the expression of Caspase-3 protein and down-regulation of the expression of Survivin.
    Correlation of P73 polymorphisms to genetic susceptibilities to cervical carcinoma and meta-analysis

    Objective To investigate the correlation of P73 G4C14-A4T14 polymorphisms in the exon 2 to susceptibilities to cervical carcinoma. Method The P73 G4C14-A4T14 polymorphisms were genotyped in 218 cervical carcinoma patients and 220 healthy controls in Chinese Hans Residing in Southern Anhui by two-step PCR-CTPP and sequencing. Meta-analysis was performed to assess the reported association between the P73 polymorphisms and cervical carcinoma. Result The distribution of P73 genotype and allele frequencies in cancer patients and controls were not significantly different. The results of genotyping by two-step PCR-CTPP were consistent with those performed by DNA-sequencing. The meta-analysis of 3 studies in Asia showed the AT allele was associated with cervical carcinoma (P<0.05). Conclusion: P73 may be a susceptibility gene for cervical cancer in Asian population, and AT alleles may increased the risk for cervical cancer.

    Objective To explore the effect of Tumor necrosis factor-a(TNF-a) and intercellular adhesion molecule-1 (ICAM-1) in cultured rat glomerular mesangial cells ( RGMC) by high glucose and intervention of Liraglutide. Methods Cultured HBZY21 RGMCs were divided into 6 groups: control cells,high glucose stimulated cells,Liraglutide low、median and high( 10、100、1000nmol/L) concentration incubated cells stimulated with high glucose,PDTC intervention cells. The expressions of TNF-a and ICAM-1 of each group were measured by ELISA and RT-PCR. Results The level of TNF-a and ICAM-1 protein or mRNA was remarkably increased by high glucose,and Liraglutide could prevent this change . Compared with high glucose stimulated cells, the expression of TNF-a and ICAM-1 protein or mRNA was decreased with PDTC intervention cells(P<0.05).Conclusions Liraglutide might play an important role in preventing diabetes nephropathy by partly relieving the expression of TNF-a and ICAM-1 induced by high glucose.
    IFNα suppresses proliferation of rat vascular adventitial fibroblast via p204

    Objective To study the basic expression of the interferon-inducible protein p204 in rat aortic vascular adventitial fibroblasts (VAFs), and the effect of interferon alpha (IFNα) on VAF proliferation and p204 expression. Methods Using immunohistochemistry and immunocytochemistry to observe the expression and distribution of protein p204, using RT-PCR and Western blot to detect the expression of induced p204 mRNA or protein. After treated with different concentrations of IFNα, the cell proliferation of the VAF was observed by MTT assay and the corresponding p204 expression was detected by Western blot. Results p204 stain was positive on rat aortic adventitia, p204 was present in the cytoplasm and also nucleus of VAF, but its expression was more obvious in the nucleus. IFNα (the terminal concentration of 1×106 IU/L) stimulating VAF more than 6 hours was enough to induce Ifi204 mRNA significant expression, and more than 24 hours for enough expression of protein p204. IFNα inhibited cell proliferation of VAF was dose dependence, followed by p204 expression increased correspondingly (P < 0.05). Conclusion There is basic expression of p204 in rat aortic VAF, p204 expression is not limited to the nucleus and IFNα can easily induce the gene and protein of p204 expression up-regulated in VAF; IFNα inhibits VAF proliferation may partly relate to the increased p204 expression.

    【Abstract】 Objective To investigate the gene mutations of beta-myosin heavy chain gene (MYH7) in Chinese pedigrees with hypertrophic cardiomyopathy (HCM), and to explore the relationship between the genotype and phenotype.Methods Standard sequence was compared with the 3, 5, 7~9, 11~16, 18~23exons of MYH7 gene which were amplified and direct sequenced from 3 chinese pedigrees with HCM to find out the mutations. Results Analyses revealed that a missense mutation of Thr441Met in exon14 was identified in one pedigree,but not in the control group.Several synonymous mutations of MYH7 gene were detected in three pedigrees. Conclusions The mutation of Thr441Met, located in the actin binding of the globular head, was first identified in Chinese. It probably caused the HCM which was a heterogeneous disease. The results suggesting that a variety of factors were concerned with the development of HCM.
    IL-6 regulates the autophagic maturation and increases the survival of PC12 treated by MPP+

    Objective To observe the effect of IL-6 on the autophagic maturation and the survival of PC12 treated by MPP+. Method IL-6 and bafilomycin A1 were administrated to PC12 cells with MPP+ treatment or not. We not only detected reproductive activity of cells with MTT method but also observed the expression of LC-3 in different conditions by Western blotting, and the formation of autophagy by transmission electron microscopy (TEM). Results After MPP+ administration, PC12 cells' autophagy level raised significantly, and bafilomycin A1 treatment slightly increased the autophagy level; IL-6 increased PC12 cells' autophagy level, but less than MPP + treatment groups, and bafilomycin A1 treatment increased the autophagy level significantly. IL-6 reduced the autophagy level of PC12 cell lines treated with MPP+, increased cell proliferation ability. Conclusion IL-6 can promote autophagic vehicles maturation, decrease MPP + induced cell injury, and promote PC12 cell survival.

    Objective To confirm the hereditary mode and the clinical manifestations of a Neurofibromatosis family with a patient develops a huge plexiform neurofibroma on the face. Methods The clinical manifestations was checked based on the standard diagnostic criteria for NF1 issued by National Institute of Health (NIH) in 1988, then the pedigree analysis and chromosome examination were carried out. Results Four clinical features of Neurofibromatosis type 1 (NF1) was shown in three of the seven members in the family. The chromosome phenotype in is normal, but Pedigree analysis showed NF1 in this family to corresponds autosomal dominant features. Conclusions Neurofibromatosis in this family is a NF1 family,and is autosomal dominant inherited disease.

    Objective To establish target SARS-CoV spike neutralizing antibody test model. Methods Construction of optimal whole SARS-CoV spike gene plasmid to induce mouse neutralizing antibody. Pretreatment with S318-510 protein targeting ACE2 to observe the inhibition change of SARS-CoV S pseudoviruses infection process via fluorescence microscope. Results SARS-CoV S1190 Abs can not only neutralize SARS-CoV pseudovirus entry into HEK293E/ACE2-Myc cells but also pretreatment with S318-510 bypass neutralization of SARS-S entry by double check. Conclusions This test model confirmation of SARS-CoV RBD targeting SARS-CoV spike neutralizing antibody may provide and explore test model of lethal viruses in laboratory without biosafety level 3.
    The efficacy of modified docetaxel, cisplatin and 5-fluorouracil combination chemotherapy in advanced gastric cancer

    Objective: To assess the efficacy of modified docetaxel, cisplatin and 5-fluorouracil (mDCF) chemotherapy in patients with advanced gastric adenocarcinoma. Methods: From January 2006, 45 patients with advanced gastric cancer were enrolled and treated with mDCF protocol. The mDCF protocol included docetaxel 60mg/m2 on d1, cisplatin 12mg/m2 on d1~5 and 5-fluorouracil 2 500 mg/m2 continuous infusion (civ) for 120h, every 3 weeks. The primary endpoint was progression-free survival (PFS) and the secondary endpoints were overall survival(OS), response rate(RR) and side effects. Results: In 40 patients who received mDCF as first-line palliative chemotherapy, no patient achieved complete response, 7(17.5%) had partial response, 9(47.5%) had stable disease, and 14(35%) showed progressive disease. Overall response rate was 17.5%(95%CI 7.5%~30.0%)and disease control rate was 65.0%(95%CI 50.0%~77.5%). Median PFS was 5.2 months(95%CI 3.6~6.8 months)and median OS was 11.0 months(95%CI 6.9~15.1months). In 5 patients who received mDCF as second-line palliative chemotherapy, 3 achieved stable disease and 2 had progressive disease. Median PFS was 5.3 months(95%CI 0~11.3 months) and median OS was 8.5 months(95%CI 0~17.1 months). Major side effects were hematologic effects. The rates of grade 3/4 neutropenia, neutropenia with fever, nausea, vomiting and diarrhea were 26.7%,11.1%, 8.9%, 8.9% and 11.1% respectively. Conclusion: The mDCF regimen is effective with a favorable toxicity profile.
    Expression of related cytokines of peripheral regulatory, CD4+effector T cells in patients with type 1 diabetes

    Objective Analyze and compare the amount of regulatory T cell and CD4+effector T cell, expression of IL-2, IL-10, IL17 and IFN-gamma in patients with type 1 diabetes (T1DM).Methods 45 T1DM, 37 T2DM patients (age ≤45 years, duration of the disease≤5 years, BMI≤30kg/m2) and 35 healthy subjects were involved. The amount of regulatory T cell and CD4+ effector T cell was analyzed by flow cytometry. Expression of cytokines such as IL-2, IL-10, IL17 and IFN-gamma were measured by real time-qPCR. Results There was negative correlation between Treg percentage and IFN-gamma expression in T1DM group (rs=-0.678, P=0.001). Positve correlation between Treg percentage and IL-10 expression was viewed in T1DM group (rs=0.773,p<0.001). Conclusion Out-of-balance of IL-10 and IFN-gamma in type 1 diabetes group suggested that the balance between preventive and destructive factors was broken.

    Objective To investigate expression of DNA methyltransferase 2 (DNMT2) protein in gastric cancer and to study proliferation of SGC-7901 (gastric cancer cell) with DNMT2 inhibition treatment (RNA interference technique). Methods Immunohistochemistry was used to detect DNMT2 expression in 60 pairs of gastric cancer and corresponding non-cancerous tissues. Immunofluorescence was used to detect DNMT2 in SGC-7901. Three siRNA (DNMT2-siRNA-1, DNMT2-siRNA-2, DNMT2-siRNA-3) targeting DNMT2 were designed. And LipofectamineTM2000 was used to transfect DNMT2-siRNA to SGC-7901. RT-PCR and western blotting were used to evaluate the effect of DNMT2 inhibition. MTT assay was carried on for SGC-7901 with DNMT2 inhibition treatment. Results Expression rate of DNMT2 in gastric cancer tissues (43.3%) was lower than that in corresponding non-cancerous tissues (93.3%), P<0.05. And DNMT2 expression was significantly lower in late-stage and poor-differentiated cancerous tissues than in early-stage and good-differentiated cancerous tissues (P<0.05). DNMT2 was distributed in nucleus of gastric cancer cell. DNMT2-siRNA could be transfect to SGC-7901 perfectly, and DNMT2-siRNA-3 could inhibit DNMT2 mRNA and protein best. At last, 96 h and 120h after DNMT2-siRNA-3 transfection, cell growth rate of SGC-7901 increased compared with control (1.138±0.110 and 1.078±0.060 vs 1, P<0.05). Conclusion Gastric cancer expressed lower DNMT2 protein than non-cancerous tissues did. Inhibition of DNMT2 could promote proliferation of gastric cancer cell more or less.

    Objective To improve the understanding of lymphoma and assess the adenosine deaminase(ADA) levels in nontuberculous pleural effusions of different actiologies. Methods A case of lymphoma with high pleural effusion ADA levels as initial symptom was retrospectively analysed,including clinical manifestation and examination. Results The male patient was 71 years old,had dyspnea for one month,with large pleural effusion in left as initial symptom.ADA levels of the pleural effusion were 620.7U/L.Finally,pleural biopsy was performed and pathology showed diffuse large B-cell lymphoma(DLBCL). Conclusion Lymphoma should be kept in mind in patients with high pleural effusion ADA levels,pathology examination should be taken in time to confirm diagnosis.

    Alpha-synuclein oligomers are the main composition of the Lewy body in patients with Parkinson’s disease, and they may be associated with oxidative stress damage and oxidative modification. The oxidative modifications include nitrative modification and dopamine oxidative adduction. Serving as a biomarker, α-synuclein may become a new target for clinical treatment.
