黑龙江省成人不同肥胖类型与心室复极不一致性的关系

Relationship between Central and Peripheral Obesity and Ventricular Repolarization Inconsistencies of Adults from Heilongjiang Province

摘要: 目的 探讨以腰围(WC)和腰臀比(WHR)为评价指标的中心性肥胖和以体重指数(BMI)为评价指标的全身性肥胖与心室复极不一致性的关系。方法 使用Philips Trim II型静息心电图仪,测量2045名黑龙江省普通成人标准12导联心电图,用Bazett公式计算校正的QT间期(QTc),设定QTc>440ms为异常。 结果 QTc和异常率在男性和女性中都存在随年龄和各项肥胖指标值增大而增加的趋势。女性显著大于男性。Logistic分析显示,年龄、性别、心率和收缩压对QTc异常有显著影响;体重指数、腰围和腰臀比也分别对QTc异常有显著影响,腰围对模型的贡献最大。结论 肥胖是影响QTc以及异常率的独立显著性因素,与心室复极不一致性有显著相关。中心性肥胖相对全身性肥胖在衡量成人心室复极不一致性上更有优势。

Abstract: Objective Investigate the relationship between central obesity, which is evaluated by waist circumference (WC) and waist-to-hip ratio (WHR), peripheral obesity, which is measured by body mass index (BMI), and ventricular repolarization inconsistencies. Methods Standard 12-lead ECGs of 2045 adults from Heilongjiang Province were measured with Philips Trim II electrocardiograph. QTc were calculated and corrected by Bazett formula. We used the criterion that QTc>440ms was abnormal. Results QTc and abnormal rates increased with age or obesity indices in both male and female. Females' QTc and abnormal rates were significantly higher than the males'. Logistic regression analysis showed that age, gender, heart rate and systolic blood pressure (SBP) had significant effect on QTc abnormal. BMI, WC and WHR also had significant effect on QTc abnormal. WC was the most significant one. Conclusion Obesity is a significant and independent factor to QTc and abnormal rates. Obesity is significantly correlated with ventricular repolarization inconsistencies. Central obesity has more advantages in weighing adults' ventricular repolarization inconsistencies than peripheral obesity.
