硬膜外分娩镇痛下产间发热产妇血中IL-6升高

IL-6 level is increased upon intrapartum fever in labor analgesia

摘要: 目的 探讨微小RNA(miRNA)在硬膜外分娩镇痛下导致产妇产间发热中的作用和分子机制。方法 分别收集硬膜外分娩镇痛并伴随产间发热(fever)、未伴随产间发热(nonfever)、以及对照产妇(control)的外周血和脐带血;ELISA检测血清IL-6水平;提取全血RNA,实时定量 PCR检测miR-146b、miR-124和miR-21的表达水平;使用miR-146b模拟物(miRNA mimic)转染293T细胞,通过双荧光报告基因试验检测miRNA对IL-6的靶向作用;Western blot检测miRNA对炎性通路的调节作用。结果 Fever 组外周血和脐带血中IL-6水平显著升高(p<0.01);而miR-146b表达水平显著降低(p<0.01);miR-146b mimic可以靶向抑制IL-6的表达,以及IL-6/Stat3炎性信号通路的活性。结论 硬膜外分娩镇痛下产妇产间发热现象与IL-6水平升高相关,并可能介由miR-146b的调控而产生。

Abstract: Objective To study whether miRNA is associated with the intrapartum fevers at term in Labor Analgesia. Methods Term cases with or without intrapartum fever after Labor Analgesia, and other cases without receiving Labor Analgesia were recruited. Maternal serum and umbilical vein blood were collected and assayed for interleukin-6 (IL-6) levels. The relative expression of miR-146b、miR-124 and miR-21 were determined by quantitative RT-PCR analysis; 293T cells were transfected with miRNA mimics or control. The effects of miR-146b on IL-6 expression were measured by dual-luciferase assay. The effects of miR-146b on inflammatory signaling pathway were examined by Western blot analysis. Results Maternal and umbilical vein IL-6 levels were higher in intrapartum fevers cases than in controls (median of 145 pg/mL vs 42 pg/mL or 37 pg/ml, p < 0.01). miR-146b expression in both Maternal and umbilical vein was decmiR-146b mimic into 293T cells reduces the reporter gene activity. miR-146b inhibits IL-6/Stat3 signaling pathway in 293T cells. Conclusions miR-146b is associated with the maternal inflammatory response after receiving Labor Analgesia via regulating IL-6 expression.
