Therapeutic efficacy of triamterene or indomethacin in patients with Gitelman syndrome

Abstract: Objective To analyze the efficacy of triamterene or indomethacin in patients with Gitelman syndrome (GS). Methods We retrospectively studied the clinical data and therapeutic efficacy of 16 patients diagnosed with GS in Peking Union Medical College Hospital from the year of 2013 to 2019. Eight patients were treated with triamterene, five patients were treated with indomethacin, and three patients were treated with both of them successively. Results Fatigue and flaccid paralysis were the first symptoms in most patients. All patients were characterized by moderate-severe hypokalemia, hypomagnesemia. Most patients had a low urinary calcium excretion, a high plasma renin activity, and metabolic alkalosis. Treatment with triamterene or indomethacin significantly increased serum potassium concentration by (0.35±0.33) mmol/L(P<0.01)and (0.46±0.48) mmol/L(P<0.05),respectively . Four patients became normokalemic with triamterene treatment, and two patients with the treatment of indomethacin. Conclusion Patients with GS usually present with typical clinical manifestations. Triamterene or indomethacin is effective for these patients.
