孕中期超敏C反应蛋白与妊娠期糖尿病相关

Association of midtrimester hsCRP with gestational diabetes mellitus

摘要: 目的 探讨孕中期超敏C反应蛋白(hsCRP)水平与妊娠期糖尿病(GDM)发病及血糖有关指标的相关性。方法 纳入GDM孕妇725人,糖耐量正常孕妇935人,详细记录受试者孕前体质量、身高、血压,于孕24-28周检测hsCRP、血糖、胰岛素水平。分析孕中期hsCRP水平与血糖和胰岛素水平及GDM发生的相关性。结果 GDM组的孕中期hsCRP水平明显高于糖耐量正常(NGT)组(p<0.01)。hsCRP水平升高是GDM发病的独立危险因素(p<0.05)。空腹血糖与hsCRP水平正相关(P<0.05),50g糖后1h血糖与hsCRP水平正相关(P<0.05),100gOGTT后1、2和3h血糖与hsCRP水平正相关(P<0.05),糖化血红蛋白(HbA1c)与hsCRP水平正相关(P<0.05),空腹真胰岛素与hsCRP水平正相关(P<0.05),100gOGTT后1、2和3h真胰岛素与hsCRP水平正相关 (P<0.05),HOMA-IR与hsCRP水平正相关(P<0.05)。结论 孕中期hsCRP水平升高能明显增加GDM的发病风险,可作为GDM诊断的指标之一 。

Abstract: Objective To explore the associations of high sensitivity C-reactive protein (hsCRP) in the second trimester of pregnancy with gestational diabetes mellitus(GDM)and glucose related traits. Methods 725 women with GDM and 935 women who remained euglycemic throughout pregnancy were enrolled in this study. Pre-pregnancy weight、height and blood pressure were recorded and hsCRP、glucose and insulin levels were detected at 24 to 28 weeks of pregnancy. The associations of hsCRP levels with the blood glucose and insulin levels were analyzed,meanwhile, we also researched the associations of hsCRP with the occurrence of GDM. Results The hsCRP levels in the second trimester of pregnancy were significantly increased in GDM subjects compared with subjects with normal glucose tolerance(NGT)(p<0.01). Elevated hsCRP was an independent risk factor for GDM(p<0.05).Fasting blood glucose was associated with hsCRP(P<0.05);1 h blood glucose after oral 50 grams of sugar was related to hsCRP(P<0.05); 1,2 and 3h blood glucose after 100 grams OGTT were related to hsCRP(P<0.05); glycosylated hemoglobin(HbA1c) was related to hsCRP(P<0.05); fasting plasma insulin was related to hsCRP(P<0.05);1,2 and 3h true insulin after 100 grams OGTT were related to hsCRP(P<0.05);HOMA-IR was associated with hsCRP(P<0.05).Conclusions The elevated hsCRP levels in the second trimester of pregnancy could significantly increase the risk of GDM,which is expected to become one of markers for the diagnosis of GDM.
