规范化培训住院医师的共情和自我效能感与客观结构化临床考试

Empathy and self-efficacy with objective structured clinical examination in residents of standardized training

摘要: 目的 了解参加住院医师规范化培训的内科住院医师共情和自我效能感的现状,并探索其心理学得分情况与客观结构化临床考试(OSCE)的相关性。方法 对北京协和医院2015级、2016级内科住院医师进行问卷调查,以医务人员的杰弗逊共情量表(JSE-HP)和一般自我效能感量表(GSES)为测量工具,并收集OSCE成绩。结果 共发放101份问卷,回收99份。内科住院医师的共情得分为(111.3 ±1.2)分,2015级住院医师得分略高于2016级,不同学制、不同来源的住院医师得分无明显差异。内科住院医师的自我效能感得分为(22.77±0.50)分,2015级住院医师得分显著高于2016级,学位较高的住院医师得分越高,本院住院医师得分高于其他住院医师。住院医师的共情得分与OSCE各项成绩无明显相关性,2015级住院医师的自我效能感得分与病历书写(R=0.35,P<0.05)、病例分析(R=0.31,P<0.05)、平均成绩(R=0.33,P<0.05)有显著正相关。结论 住院医师的共情和自我效能感有待提高,可通过加强临床培训予以提升。住院医师规范化培训体系引入心理学测评,可完善OSCE对住院医师的综合考评,有助于改进规范化培训体系。

Abstract: Objective The purpose of this study was to investigate the current situation of empathy and self-efficacy, and relation with performance of Objective structured clinical examination in residents of standardized training. Methods Questionnaire surveys with Health Professional-version of Jefferson Scale of Empathy and General Self-Efficacy Scale were conducted among residents of grade 2015 and grade 2016 from Dept. of Internal Medicine of PUMCH. Results 101 questionnaires were delivered, and 99 were effectively received. The average empathy score was 111.3 ±1.2, with that of grade 2015 slightly higher than that of grade 2016. The empathy score from residents of different degree and different sources showed no significant difference. The average self-efficacy score was 22.77±0.50. The score of residents of grade 2015 was significantly higher than that of grade 2016. The score was higher in residents with higher degree. The score of residents from PUMCH was higher than the other subgroups. The score of empathy showed no significant correlation with OSCE scores, while the score of self-efficacy of residents of grade 2015 significantly positively correlated with scores of medical recording (R=0.35, P<0.05), case analyzing (R=0.31, P<0.05) and average score(R=0.33, P<0.05) of OSCE. Conclusions The empathy and self-efficacy of residents remained to be improved, and could be improved through clinical training. Psychological evaluation could be inducted into standardized resident training system, and provide helpful supplementary to OSCE with more comprehensive evaluation of residents.
