卵泡液促甲状腺激素水平对辅助生殖技术实验室指标的影响

Influence of follicular fluid thyroid stimulating hormone on laboratory indicators of assisted reproductive technology

摘要: 目的 评估卵泡液(FF)中甲状腺素(TH)水平和促甲状腺激素(TSH)水平之间的关系,并确定TSH是否影响辅助生殖技术实验室指标及临床结局。方法 142例实施体外受精-胚胎移植(IVF-ET)患者,根据卵泡液中TSH浓度分为TSH<2.5 mIU/L(A组),2.5≤TSH<4 mIU/L(B组)和TSH≥4 mIU/L(C组)3组。月经第2~3天留取血液标本,取卵日留取第一管清亮、无血液污染的卵泡液,采用电化学发光法检测卵泡液TSH、游离四碘甲状腺原氨酸(fT4)和游离三碘甲状腺原氨酸(fT3)水平。结果 1)A组获卵数显著低于B组和C组(P<0.05),B组获卵数显著低于C组(P<0.05);C组MII率显著低于A组和B组(P<0.05);B组ICSI受精率显著高于A组和C组(P<0.05);C组可利用胚胎率显著低于A组和B组(P<0.05);但C组可利用囊胚率显著高于A组和B组(P<0.05)。2)卵泡液中TSH浓度与血液中TSH浓度呈正相关(P<0.05),卵泡液中TSH浓度与fT3浓度呈正相关(P<0.05)。结论 卵泡液中TSH水平和血清基础TSH水平呈正相关,TSH≥4 mIU/L可能会影响辅助生殖技术的实验室指标。

Abstract: Objective To evaluate the association between thyroxin(TH) level and thyroid stimulating hormone (TSH) in follicular fluid (FF) and in serum and whether TSH affects laboratory indicators predicting assisted reproductive technology outcomes. Methods This study enrolled 142 women undergoing ART. According to the concentration of TSH in follicular fluid, the volunteers were divided into three groups: TSH<2.5 mIU/L (group A), 2.5≤TSH<4 mIU/L (group B) and TSH≥4 mIU/L (group C). Blood samples on day 2 or 3 of menstruation and the first tube of clear, blood free follicular fluid on the day of retrieval oocytes were collected to detect the levels of TSH, fT4 and fT3 with electrochemical luminescence method. Results 1)The average number of eggs obtained in group A was significantly lower than group B and group C, that of group B was significantly lower than that in group C (P<0.05); The rate of MII in group C was significantly lower than that in group A and group B(P<0.05); The fertilization rate of ICSI in group B was significantly higher than group A and group C (P<0.05);The available embryo rate in group C was significantly lower than group A and group B (P<0.05); However, the blast cyst utilization rate in group C was significantly higher than that in group A and group B (P<0.05). 2)There was a positive correlation between TSH concentration in follicular fluid and blood concentration (P<0.05), and between TSH concentration in follicular fluid and fT3 concentration (P<0.05). Conclusions TSH level in follicular fluid is positively correlated with TSH level in basal serum. TSH ≥ 4 mIU/L in follicular fluid on the day of oocyte retrieval may have some effects on ART outcome.
