Buprenorphine transdermal patch reduces perioperative opiate consump-tion in thoracoscopic surgery

摘要: 目的 观察丁丙诺啡透皮贴剂用于胸腔镜术后镇痛的临床效果和安全性。方法 将100 例拟行胸腔镜手术的临床I期肺癌患者随机分为观察组51例采用丁丙诺啡透皮贴进行术后镇痛,对照组49例采用氨酚羟考酮进行术后镇痛,术后均给予帕瑞昔布钠联合镇痛。观察两组患者术后疼痛情况、对镇痛的满意度以及不良反应发生情况。结果 术后3d内,观察组术后静息痛和运动痛程度均低于对照组(P<0.001),观察组换算的吗啡用量远低于对照组(P<0.001),而观察组患者满意度高于对照组(P<0.001)。对照组总的不良反应发生率高于观察组(P<0.001)结论 丁丙诺啡透皮贴剂用于胸腔镜术后镇痛效果良好,且能减少围术期阿片类药物用量。

Abstract: Objective To observe the clinical effect and safety of buprenorphine transdermal patch for analgesia after thoracoscopic surgery. Methods 100 patients with stage I lung cancer undergoing thoracoscopic surgery were randomly divided into observation group (51 cases) and control group (49 cases). The observation group was treated with buprenorphine transdermal patch for postoperative analgesia, while the control group was treated with oxycodone paracetamol for postoperative analgesia. Both groups were treated with parecoxib sodium for combined analgesia. The postoperative pain, satisfaction with analgesia and the occurrence of adverse reactions were observed in both groups. Results Within 3 days after operation, the degree of resting pain and motor pain in observation group was lower than that in control group (P < 0.001), the amount of morphine converted in observation group was much lower than that in control group (P< 0.001), and the satisfaction of patients in observation group was higher than that in control group (P < 0.001). The overall incidence of adverse reactions in the control group was higher than that in the observation group (P< 0.001). Conclusions Buprenorphine transdermal patch has good analgesic effect after thoracoscopic surgery and can reduce the amount of opioids used in perioperative period.
