微卫星不稳定、HER2与结直肠癌恶性程度相关

摘要: 目的 探究结直肠癌中微卫星不稳定(MSI)、人表皮生长因子受体2(HER-2)表达情况与其恶性程度的相关性,以及二者联合检测在结直肠癌中可能的临床应用价值。方法 对大连医科大学附属第二医院胃肠外科诊治的425例术前未接受任何形式辅助治疗的结直肠癌患者临床资料进行回顾性分析。并应用统计学方法对MSI、HER2表达情况与临床资料间相关性进行分析。结果 结直肠癌中HER2高表达与肿瘤分化程度、侵袭深度相关(P<0.05);MSI-H与MSS/MSI-L在肿瘤大小、部位、分化程度、组织类型、侵袭深度、贫血和CA199表达方面存在差异(P<0.05)。MSI与HER2表达情况存在相关性(P<0.05)。结论 HER2、MSI在结直肠癌恶性程度评估方面具有一定协同性,将两者联合检测对结直肠癌恶性程度评估或有一定指导作用。

Abstract: Objective To investigate the correlation between microsatellite instability (MSI) and human epidermal growth factor receptor 2 (HER-2) expression with its malignancy in colorectal cancer, and the clinical application value of the combination of MSI and HER-2 in colorectal cancer. Methods A retrospective analysis was performed on the clinical data of 425 colorectal cancer patients who did not receive any form of adjuvant therapy before the treatment of gastrointestinal surgery in the Second Affiliated Hospital of Dalian Medical University. Statistical analysis was carried out to analyze the correlation between MSI and HER2 expression with its clinical data. Results The high expression of HER2 in colorectal cancer was related to the degree of tumor differentiation and depth of invasion (P<0.05). MSI-H and MSS/MSI-L were related to tumor size. There were differences in the site, differentiation, tissue type, depth of invasion, anemia, and CA199 expression (P<0.05). There was a correlation between MSI and HER2 expression. Conclusions HER2 and MSI have certain synergy effects in evaluating the degree of malignancy of colorectal cancer. The combined detection of these two factors could have an important guiding role in the assessment of malignancy of colorectal cancer.
