住院患者择期手术当日取消原因分析

Causes for cancellation of inpatients elective operations on the day of surgical operation

摘要: 目的 分析住院患者择期手术取消原因,并提出可能改进的措施,为提高医疗资源利用率和患者满意度提供参考。方法 通过医院相关电子信息系统(HIS),回顾性分析北京协和医院2019年1月1日至2020年7月31日住院患者中择期手术取消的病例资料,统计取消百分比率并分析取消原因。结果 在66 090例择期手术住院患者中,343例(0.52%)当日取消手术,取消原因可以分为疾病因素(21.3%)、手术相关因素(39.7%)、患者因素(29.2%)、以及麻醉相关因素(9.9%)等4类;针对各种因素具体分析,提出可能改进的措施。结论 通过对各相关因素的分析,发现大部分择期手术的取消可通过加强术前评估等相关措施得以避免,从而降低择期手术取消率。

Abstract: Objective To explore the rate and causes for cancellation of inpatients elective operations on the day of surgical operation in order to develop strategy for optimization of resource utilization. Methods This study retrospectively investigated inpatient elective operation cases in Peking Union Medical College Hospital from January 1 2019 to July 31 2020 from the hospital electronic information system and analyzed the rate and causes for cancellation of elective operations on the day of surgery. Results Totally 343 cases (0.52%) cancelled operation on the day of surgery among 66 090 inpatient elective operations. Reasons for cancellation involved medical(21.3%), surgical(39.7%), patients(29.2%)and anesthesia(9.9%)factors. Conclusions Most cancellation events can be prevented by improving specific measures like dedicated preoperative anesthesia assessment and the rate of operation cancellation is expected to be reduced significantly.
