目的 调查某儿童医院超说明书用药情况,为加强儿童用药安全管理提供依据。方法 采取回顾性分析的方法,随机抽取该儿童医院2021年1月至12月的门诊处方进行点评,根据药品说明书对处方的超说明书用药行为进行判断及归类。结果 共调查有效处方2 514张,含医嘱7 159条,涉及264种药品,超说明书用药发生率按处方数为57.24%,按医嘱条数为27.34%。主要超说明书的类型为未提及儿童用药信息(57.23%)和超适应证或功能主治(25.86%)。结论 该院门诊超说明书用药现象普遍,多种药物缺乏关于儿童使用的信息。建议医院结合国家最新法律政策,完善超说明书用药管理制度,细化儿科超说明书用药规范,确保儿童用药安全。
Abstract
OBJECTIVE To investigate the off-label drug use in hospitals and provide data for strengthening the safety management of children's medication. METHODS By retrospective analysis,the outpatient prescriptions of the children's hospital from January to December in 2021 were randomly selected for comments, and the off-label drug use behaviors of prescriptions were judged and classified according to the drug instructions. RESULTS A total of 2 514 effective prescriptions, including 7 159 medical orders, involving 264 drugs were investigated. The incidence of off-label drug use was 57.24% according to prescription and 27.34% according to medical orders. The main types of off-label were no information on medication for children (57.23%) and off-label or functional indications (25.86%). CONCLUSION Off-label drug use is commonly seem in the outpatient department of the hospital, with a lack of information about the use of various drugs in children. It is suggested that the hospital should improve the management system of off-label drug use in accordance with the latest national laws and policies, and specify the specifications of off-label drug use in pediatrics, so as to ensure the safety of drug use in children.
