摘要

目的 探讨 <> β 受体阻滞剂对盐酸去甲乌药碱对心率、血压作用的影响;以及冠状动脉狭窄后药物负荷实验结果。 方法 ■ 10 只中华小型猪,随机分别注射选择性 <> β 1 受体阻滞剂酒石酸美托洛尔、非选择性 <> β 受体全阻滞剂普萘洛尔部分或完全阻断 <> β 受体后,再注射盐酸去甲乌药碱,记录试验猪的心率、血压的变化;并与单独注射盐酸去甲乌药碱时心率、血压变化进行比较。试验猪造成慢性冠状动脉狭窄模型后,进行盐酸去甲乌药碱药物负荷试验,静态心肌 SPECT 显像。 结果 在基础状态,静脉注射盐酸去甲乌药碱后,试验猪平均心率最大增加( 40±17 )次 ·min - 1 (<>P<0.001) ,平均收缩压变化没有统计学差异,而平均舒张压最大变化( 20±16 ) mmHg(<>P=0.003 5) 。美托洛尔 <> β 1 受体和普萘洛尔全 <> β 受体阻断后,试验猪平均心率最大增加分别为( 27±17 )和 (8±9) 次 ·min-1 ,与基础状态相比,差异有统计学意义 ( 分别为 <> P = 0.000 3 和 <> P = 0.022 1) 。 冠状动脉狭窄猪的心率在 药物负荷后心率增加有统计学意义 (<>P<0.05) 。 应用盐酸 去甲乌药碱负荷试验放射性核素心肌显像,检测冠状动脉病变的敏感性约为 90% 。 结论 本试验结果提示,盐酸去甲乌药碱可能部分通过 <> β 受体发挥其在心血管系统的作用,并可被 <> β 受体阻滞剂所部分阻断;作为负荷药物可用于心肌灌注显像中检测冠状动脉狭窄和心肌缺血。

Abstract

OBJECTIVE To investigate the effect of hydrochloric higenamine on cardiovascular system after the administration of β-receptor blocker and to measure the result of stress experiment in coronary artery stenosis model. METHODS ■Selective β1-receptor blocker metoprolol tartrate and non-selective β-receptor blocker propranolol were intravenous injected in ten Chinese miniature pigs, respectively. Heart rate (HR), systolic blood pressure(SBP) and diastolic blood pressure(DBP) were recorded after higenamine was introduced, and compared with those treated with higenamine along. Drug stress test and static myocardial SPECT imaging in model of chronic coronary artery stenosis were performed. RESULTS Mean HR of pigs was increased (40±17) beat·min-1 (P<0.001), mean DBP was decreased (20±16) mmHg (P=0.003 5), SBP showed no significant change (P>0.05) after intravenous administration of higenamine. Mean HR of pigs was also increased by higenamine after metoprolol and propranolol intervention (27±17) and (8±9) beat·min-1, respectively. Compared with baseline, change of HR was significant (P=0.000 3; P=0.022 1, respectively). The sensitivity for the detection of coronary artery disease by radionuclide myocardial perfusion imaging with higenamine was 90%. CONCLUSION This study demonstrated that active β-receptor blocker, at least partially, contributed to effect on cardiovascular system by hydrochloric higenamine, β-receptor blocker could partially block effect and myocardial perfusion imaging with higenamine had high sensitivity for detecting coronary artery stenosis and ischemic myocardium.
