药物性肝损伤的免疫学机制

摘要

目的从免疫学的角度出发,探讨药物性肝损伤(drug-induced liver injury,DILI)的发病机制。方法查阅近年相关研究,对文献资料进行分析和总结。结果DILI的遗传特异性和普遍的不良预后情况使得DILI成为药物研发中的最重要的安全性问题之一;同时,DILI也是许多上市药物撤出市场的主要原因。DILI最初是由药物或其代谢产物的直接肝毒性作用引起的。肝实质细胞的损伤激活免疫细胞,促使其释放炎症因子或肝毒性介质,并可启动针对药物相关性抗原的免疫应答。结论肝脏免疫系统独特的成分和反应特性是揭示DILI发生机制的基础,只有从细胞和分子水平上了解这些基础,才能对DILI作出有效的预测和防治。
