摘要

目的利用半定量逆转录-聚合酶链式反应(RT-PCR)技术,测定细胞因子白细胞介素-2(IL-2)、白细胞介素-4(IL-4)、干扰素-γ(IFN-γ)mRNA表达水平,研究其在肾移植患者术后,钙调磷酸酶抑制剂治疗的疗效评价中的应用。方法取肾移植后患者外周血分离淋巴细胞,提取总RNA,进行RT-PCR反应,通过光密度扫描进行半定量分析;结合治疗药物监测结果分析。结果他克莫司(FK506)组3种细胞因子表达显著低于环孢素(CsA)组,且离散程度小。血药浓度谷值(ρ0)与细胞因子表达相关性不强。CsA组3种细胞因子之间的相关性比FK506组好。尿蛋白阳性的患者较阴性患者血清肌酐(Cr)显著升高,且IL-4表达显著升高。结论钙调磷酸酶抑制剂的个体敏感性差异很大。IL-2,IL-4,IFN-γmRNA的表达可以作为血药浓度的重要补充,为个体化给药提供依据。

Abstract

OBJECTIVE To measure interleukin-2(IL-2),interleukin-4(IL-4),interferon-γ(IFN-γ) mRNA level with semi-quantitative reverse transcription polymerase chain reaction(RT-PCT) technology,and evaluate the calcineurin inhibitors(CIs) therapeutic effect after renal transplantation.METHODS The leukomonocyte was separated from renal transplant recipient peripheral blood,RNA was isolated before RT-PCR reaction.The quantitation was obtained by densitometric scan of agarose gel electrophoresis photos.The results were analyzed with drug concentration.RESULTS The expression of 3 cytokines of Tacrolimus(FK506) group was much lower than those of Cyclosporine(CsA) group,furthermore the coefficient of variation(CVs) was less.ρ0 was not well related with cytokine level.The correlations of 3 cytokines of CsA group were better than those of FK506 group.Creatinine(Cr) was elevated in patients with positive proteinuria,and their IL-4 mRNA expression was increased.CONCLUSION The CIs have great sensitivity differences among individuals.IL-2,IL-4 and IFN-γ mRNA expression would be the assistant of CIs' blood concentration,consummating the individualized administration.
