Abstract:

Migraine is increasingly recognized as a disorder involving neurovascular dysregulation and large-scale brain network dysfunction. However, because of its substantial clinical heterogeneity, diagnosis and subtype classification still rely predominantly on symptom-based criteria, and specific biomarkers remain unavailable. The identification of objective, reproducible, and quantifiable imaging markers has become a major priority in both migraine research and clinical translation. By integrating structural MRI, diffusion MRI, resting-state fMRI, and perfusion imaging within a unified framework, multimodal MRI enables a comprehensive evaluation of the interplay among structural alterations, functional reorganization, and cerebral perfusion changes. This multimodal approach provides more robust evidence for elucidating the pathophysiological mechanisms of migraine, characterizing subtype-specific neuroimaging features, and clarifying processes related to chronification. In this review, we summarize recent advances and potential clinical applications of multimodal MRI in migraine from three perspectives: brain structure, brain function, and structure-function coupling.
